Our Process and requests:

• Review our policy and financial agreement with your patient to ensure we are a good fit for their treatment plan

• Accurately fill out our referral form through our secure portal with the patient’s information, and specific reasons for the referral. Please include medical history, psychiatric history and current medications when necessary.

• Direct patient to our schedule to find the next available time for an initial psychiatric evaluation
